Placement of Stents During Surgery
The enlargement of stenotic blood vessels by patching during surgery is challenging and not always effective. Also, it requires extended circulatory arrest and cardiopulmonary bypass while the patching is performed, with increased risk to the patient.
In the hybrid approach, the stenotic vessels may be seen during surgery and enlarged by transcatheter stent placement. The catheterization procedure, assisted by direct vision, is very quick and much more effective than the surgical approach.
When possible, stenting is performed during catheterization procedures without the need for surgery. However, the hybrid approach is used if surgery is necessary for other reasons, if vascular access for catheterization is limited, or if complications during transcatheter stenting make surgery necessary.
Some examples of stent placement during surgery:
- Stenting of pulmonary arteries during surgery
- Stenting of a coronary artery during the Fontan procedure
- Stenting of the superior vena cava during heart transplant surgery
- Stenting of coarctation of the aorta during hybrid stage 1 palliation for complex congenital heart disease |
